1.      Birth Certificate of Child. ( Show Original & take Xerox Copy)

2.      Aadhar Card of Child. ( Show Original & take Xerox Copy)

3.      Aadhar Card of Guardian. ( Show Original & take Xerox Copy)

4.      2 Copy photo of Child.

5.      Phone number of Guardian.

6.      Signature of Guardian.
